As an HR Professional you should be familiar with OSHA forms for maintaining employee records. Which OSHA form is used to cover the what, how, when, where, and who or work- related injuries?

Form 301, the Injury and Illness Incident Report, is a supplementary form providing additional detail on individual incidents - it is not the main log used to track all recordable events.

OSHA Form 300, the Log of Work-Related Injuries and Illnesses, is the primary employer recordkeeping form that captures details of each recordable incident including the employee affected (who), the nature of the injury (what), the date (when), the location (where), and the circumstances (how). Employers with 11 or more employees must maintain this log throughout the calendar year.

Form 300A is the annual Summary of Work-Related Injuries and Illnesses, which aggregates yearly totals and must be posted but does not capture individual incident details.

Form 3165 is the OSHA 'Job Safety and Health: It's the Law' worker rights poster, not an injury and illness recordkeeping form.
