Why each option
When installing a managed package in a sandbox, the installation URL must be modified to 'test.salesforce.com', and the package will be removed upon sandbox refresh.
Metadata changes to a managed package itself are not recreated; any custom configurations built *on* the package would need to be deployed to production, not the package's core metadata.
The installation URL for AppExchange packages must be changed from 'login.salesforce.com' to 'test.salesforce.com' to install into a sandbox environment.
All installation options (Admins Only, All Users, Specific Profiles) are available when installing a managed package into a sandbox, just as in production, to allow for comprehensive testing of user access.
Sandbox refreshes overwrite the existing sandbox with a fresh copy of the production organization's (or template's) metadata and data, thereby removing any packages or configurations unique to that sandbox.
